Required Documents for Organ (Kidney/Liver) Transplantation NOC

Following documents are required to obtain an NOC for organ (Kidney/Liver) Transplantation:

1. 01 (one) copy of duly filled in application form (Form 21)

2. Request Letter from the Hospital

3. Affidavit from Patient, Donor and their guardians (Must be attested by the Ministry of Foreign Affairs in Dhaka)

5. If the Guardians of the Donor/Recipient (if unmarried, then Parents)/ (if married, then Spouse) is dead and Donor/Recipient wants to use another person as Guardian, then death certificates need to be submitted (Must be attested by the Ministry of Foreign Affairs in Dhaka)

6. Family Tree/Certificate of the relationship between the donor and the patient (Must be attested by the Ministry of Foreign Affairs in Dhaka)

7. Marriage certificate/Nikahnama of donor in case the donor is married.

8. Copies of Passport & Visa of the patient, donor and guardians (Must be attested by the Ministry of Foreign Affairs in Dhaka)

9. Fees: No Fees Applicable

Note: With above documents Guardian/Attendant of the Patient and Donor (03-02 persons) need to visit to this Mission personally with original passports and NIDs for interview. Depending on the interview, the Mission may ask for additional documents.
